This program is designed to give you moresupport between your regular doctor visits. As a participant, you can expect:

•         Regular check-ins from a dedicated care teammember who is familiar with your care

•         Help manage your medications and make sure yourcare plan is working for you

•         Coordination between your providers so nothingfalls through the cracks

•         A direct line to someone you can reach whenquestions or concerns come up, without waiting for your next appointment

•         Proactive outreach to help catch potentialissues early, before they become bigger problems
